Application to the Continuing Education Program - Puerto Rico Conservatory of Music

I certify that all information provided in this application is correct, truthful, and complete. I acknowledge that fabricating and/or providing incorrect information may be considered just cause for denial of admission or suspension from the Conservatory of Music of Puerto Rico. Terms: 1) A current invoice is required for payment. 2) Payments must be made for the full amount reflected on the invoice. Failure to do so will result in an additional $25 charge for an extension. The outstanding balance will be debited to the card on the payment plan date. 3) Only tuition payments will be accepted through this method. 4) The charge is subject to credit availability.
